If you are unable to do this please visit reception between 9 am to 5 pm and collect a paper registration form.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

The following ID and proof of address are also required:

Adults: photographic ID (passport, driving licence etc,) AND a utility bill/council tax bill/driver’s licence; anything official generated in the last 3 months; it must include your current address.

Children: dependent on age, Red Book, birth certificate, or passport if 16 or over.

Named GP

All patients at Rosegrove Surgery have a named GP who is responsible for your overall care. You can contact the practice to find out who your named GP is. Having a named GP does not prevent you from seeing any other GP or clinician in the practice.

Accessing someone else's information

You can view parts of your medical record online through the NHS App or Patient Access. This includes test results, allergies, immunisations, and consultation notes.

You can also nominate someone else (a proxy) to have access to your records if required. Please contact reception for more information and to complete the necessary consent forms.
